I agree to comply with all policies & procedures and to support the mission of ABSPI. To serve without remuneration for my services. I understand I am not an employee, I am a volunteer, ABSPI can terminate my volunteer status at any time, with or without cause. I also understand that, while volunteering I acknowledge that participant's information is confidential and cannot be disclosed, shared or repeated. You are entering an area where photography, audio, and video recording may occur. I acknowledge & grant permission to be photographed, interviewed, audiotaped or videotaped, which will be used as it deems fit, in ABSPI sole discretion. I understand ABSPI shall own all intellectual property rights, if any, arising in connection therewith, and I further agree and acknowledge that I shall not receive (or be entitled to receive) any compensation from ABSPI for any such usage. I certify the information provided on this form is true and complete to the best of my knowledge and belief; I have not knowingly withheld any pertinent facts or circumstances. I understand any falsifications, misrepresentation or omissions by me in connection with this application may result in my dismissal from the volunteer program.
